Configuring the form.
As a project Administrator or Site Administrator you need to open project Setting in that you can see the option of Secure form when setting.
When you will click on that You it, you can see the list of Secure form which is configured in this project. Here you , if you have already created one. You can see Each each form will be linked with One one Issue-type and We we cannot link two forms with Same same Issue-type (as of now).

This Secure Form builder provides multiple type of fields support like Header text, Label, Paragraph, Date, Line Break, Dropdown, Tags, Checkboxes, Rating, Multiple choices, Text, number & multi line input, Radio Type, select field and File Attachment etc.
Using this many fields type in a single form makes the form simpler and easy to understand.

All you need to do is just drag and drop the required field type in the form and you are good to go.
